St. Croix Hospice Earns Recognition for Quality Connections

In a significant achievement, St. Croix Hospice has been honored for the fifth consecutive year by attaining four rings in the prestigious 2025 Quality Connections program, organized by the National Alliance for Care at Home. This program evaluates hospice providers on their commitment to excellence in care through a comprehensive framework focusing on four key pillars: Education, Application, Measurement, and Innovation.

These pillars serve as a foundation for participants who engage in critical activities, including quarterly data submissions, benchmarking, educational courses, and involvement in case studies aimed at addressing emerging issues that impact hospice care. The culmination of a provider's engagement across these areas results in the potential closure of all four rings, a measure of the quality of care they deliver.
