Introduction
CrossLog Inc., headquartered in Hakata, Fukuoka, led by CEO Tomoharu Miyahara, has announced the release of new functionalities for its home medical care DX support service, "CrossLog." These features are designed to assist healthcare providers in adapting to the 2026 medical fee revision's increased demands on quality in home medical care.
Background
The 2026 medical fee revision emphasizes strict quality standards for home medical treatment. Among these changes, the newly established "Home Medical Care Enhancement System Addition" has seen significant increases in points awarded compared to the previous system, which included the "Home Palliative Care Enhanced Clinic and Hospital Addition." However, the requirements to qualify have become more demanding, posing challenges for numerous medical institutions.
Furthermore, the criteria for calculating the General Management Fees for Home Medical Care (in-home management) and the Facility General Management Fees have been revised, increasing the need for close monitoring of patient visit frequency based on the severity of their conditions. New expectations, such as developing and regularly reviewing a Business Continuity Plan (BCP), have also been added to the baseline requirements for facilities providing home medical care.
To alleviate the increased operational burdens on healthcare professionals and ensure they can effectively utilize necessary information in their daily operations, CrossLog has introduced these new features.
New Features of CrossLog Addressing the 2026 Medical Fee Revision
1. Automated Calculation of Severe Patients and Facility Threshold Evaluation
Based on scheduled home visits and patient data registered within CrossLog, required information can be automatically compiled on the CrossLog report interface. This allows for instant evaluation of the new "Home Medical Care Enhancement System Addition" and the tweaked standards for "General Management Fees (in-home management and facility)." This automation reduces manual data gathering efforts needed for eligibility confirmation while supporting ongoing monitoring capabilities crucial for compliance.
- - Home Medical Care Enhancement System Addition: In addition to existing aggregation items such as end-of-life care and emergency visit performance, the volume of patients meeting the new criteria listed in "Table 8-2" will be auto-calculated.
- - General Management Fees: The ratio of severe patients according to the facility standards—those requiring more than two visits monthly—will also be determined through automated compilation processes.
2. Hazard Map Functionality
In response to the newly required "BCP creation and periodic review" for facilities providing home medical care, CrossLog has implemented a hazard map feature within its patient mapping tools. This allows users to view patient locations alongside disaster risk data directly on the platform, greatly easing the burden of BCP planning.
- - Disaster Risk Visualization: Users can overlay regional hazard maps onto the patient location map with a click.
- - Priority Clarification: Users could refine their searches based on medical dependency and other criteria on the hazard map, aiding in the prioritization of responses during emergencies.
Support for ICT Integration and Online Medical Consultation
Beyond the new functionalities mentioned above, CrossLog is equipped to support the promotion of online medical consultations and multi-professional collaboration using ICT—an essential focus of the current fee revision. All solutions are designed with appropriate functionalities and security measures to ensure safe usage.
1. Video Call Feature
Users can initiate video calls from the CrossLog calendar, handling both starting and notifications through one centralized system. This seamless process facilitates efficient online consultations without needing app download.
2. CrossLog Connect Multi-Professional Collaboration App
With the new revisions clarifying ICT options eligible for Home Medical Information Collaboration Costs, the CrossLog Connect app enables smooth communication across various professionals. By centralizing data from CrossLog and external tools, it allows for tighter coordination and swifter responses in care delivery.
