Cost Neutrality of Blue Light Cystoscopy vs. White Light Cystoscopy



A groundbreaking study published in JU Open Plus by Photocure ASA has shed light on the cost implications of using Blue Light Cystoscopy (BLC) compared to White Light Cystoscopy (WLC) for non-muscle-invasive bladder cancer (NMIBC) diagnosis. Conducted in the U.S. using data from the Optum Research Database, the study analyzes healthcare resource utilization and costs associated with both procedures.

Bladder cancer is one of the most prevalent malignancies worldwide, significantly contributing to patient morbidity and economic burden. The aim of this research was to evaluate the economic implications of BLC in real-world settings where it has shown improved detection rates of carcinoma in situ (CIS) compared to WLC.

Research Methodology


The researchers employed a retrospective cohort analysis, examining claims data from patients who underwent either BLC or WLC between June 2011 and May 2023. A total of 794 patients were in the BLC group, compared to 4,764 matched patients in the WLC cohort. The analysis focused on various economic measures, subscribing to a per-patient-per-month (PPPM) framework in inflation-adjusted 2023 USD.

Key Findings


The analysis revealed that during the baseline period, BLC patients exhibited a greater prevalence of CIS detection than WLC as per reimbursement claims. Additionally, BLC patients tended to undergo more upper tract imaging and biomarker testing. Despite these increased healthcare demands, the results showed that the total healthcare costs were not significantly different between the two groups. Specifically, the all-cause costs were $2,987.93 for BLC patients compared to $2,886.16 for those undergoing WLC (p=0.65). Bladder cancer-specific costs also proved comparable, at $1,301.10 for BLC and $1,246.60 for WLC (p=0.75).

The authors concluded that while BLC fosters enhanced detection of CIS, resulting in increased frequency of surveillance and intervention, it does not contribute to a rise in overall healthcare costs when evaluated with an Inverse Probability of Treatment Weighting (IPTW) model and variable patient follow-up. This positions BLC as a cost-neutral yet valuable diagnostic tool for managing NMIBC.

Implications and Future Research


The findings from this study are promising and significantly contribute to the literature supporting the clinical utility of BLC for NMIBC patients. Mark Tyson, the principal investigator of the study, expressed that “BLC will continue to play a crucial role in enhancing the quality of diagnostics and patient care in NMIBC.” He emphasized the importance of strategies that maximize diagnostic accuracy in initial assessments and follow-ups, aiming to optimize costs while improving patient outcomes.

As bladder cancer continues to present a significant challenge in public health, further investigation into the long-term oncologic and economic impacts of BLC is warranted. This clinical inquiry will be crucial for guiding future practices and policies in the management of bladder cancer, particularly given its costly nature and the necessity for frequent surveillance due to high recurrence rates.
