National Skilled Nursing Care Week Begins Sunday
National Skilled Nursing Care Week kicks off this Sunday, marking an important annual tradition that honors the essential roles of skilled nursing facilities and the invaluable work of their employees and volunteers. The week is dedicated to fostering connections among residents, families, and caregivers, encapsulated in this year's theme, "Tapestry of Life."
Celebrating Connections in Care
Emphasizing the significance of relationships within skilled nursing, the theme aims to shine a spotlight on the myriad stories and experiences shared by those involved in the care process. The Michigan Health Information Network Shared Services (MiHIN) is proud to contribute to this mission by enhancing healthcare for residents throughout the state.
MiHIN's Role in the healthcare landscape is fundamental. The organization has established secure networks connecting hospitals, pharmacies, payers, and skilled nursing facilities, ensuring that quality care is accessible to all. Their collaborative model uplifts patient care by leveraging technology and facilitating essential health information exchanges.
In a significant stride towards improving care quality, MiHIN collaborated with PointClickCare in September 2022. This partnership empowers over 100 skilled nursing facilities in Michigan to obtain vital patient information seamlessly.
Dr. Isabell Pacheco, Chief Administrative Officer at MiHIN, articulates the impact: "This collaboration significantly enhances the care provided to Michigan's elderly and disabled populations. By delivering real-time information about admissions, discharges, and transfers, we can address care coordination gaps across different providers. It also enriches the data available to care teams and health systems."
Advancements Through Partnership
Thanks to the MiHIN and PointClickCare partnership, skilled nursing facilities experience several notable benefits that bolster the quality of care:
- - Enhanced Care Coordination: Providers can track patient transitions as they happen, resulting in improved continuity of care.
- - Better Post-Acute Care Transitions: Timely information about admissions and transfers helps mitigate care gaps, allowing for faster follow-up actions and more informed decision-making.
- - Stronger Health Data Networks: With the integration of these facilities, MiHIN has broadened its role as a health information hub across Michigan, bridging essential data gaps in patient transitions.
Hamad Husainy, Chief Medical Officer at PointClickCare, states, "By sharing data with MiHIN, we enhance the healthcare data ecosystem, ultimately leading to improved care quality and reduced costs from visibility gaps in patient transitions."
Join the Celebration!
From May 11 to May 17, National Skilled Nursing Care Week will celebrate the vital contributions of caregivers across the state. This week is an opportunity for communities to come together, recognize those serving in skilled nursing roles, and share their stories of compassion and commitment.
