Introducing the 2025 Pharmacists' Patient Care Process
The Joint Commission of Pharmacy Practitioners (JCPP) has announced the updated
Pharmacists' Patient Care Process (PPCP) for 2025. This revision, building on the previous 2014 version, aims to reflect the evolving landscape of pharmacy practice and the critical role pharmacists play in healthcare today. The updated PPCP provides a comprehensive framework designed to emphasize
person-centered care, which prioritizes the individual needs of patients across various pharmacy settings.
A Collaborative Effort for Improvement
This 2025 update is a product of an extensive two-year collaboration, involving representatives from
14 national pharmacy organizations. In June 2023, a dedicated Patient Care Process Workgroup (WG) was formed to comprehensively assess and rework the PPCP. By utilizing a consensus-driven modified Delphi method, the WG diligently reviewed the original PPCP, incorporating a decade of practical feedback from pharmacy practice, education, and training initiatives. Significant shifts in the healthcare sector and evolving pharmacist roles in patient care were also considered in the revisions. A public comment period allowed wider input from the pharmacy community, informing the final updates that were ultimately approved by the JCPP CEOs.
Emphasizing Pharmacists' Expertise
A central theme of the revised PPCP is a renewed focus on the
medication expertise of pharmacists. This comes in response to considerable changes in healthcare delivery systems and the ever-expanding functions of pharmacists. Some key considerations guiding the revision include:
- - Adoption of person-centered terminology.
- - Addressing health equity, health disparities, and the impact of social determinants on health.
- - Changes that reflect the evolving scope of practice for pharmacists.
- - Recognition of emerging patient care delivery modes.
- - A concerted emphasis on collaboration and coordination with patients, caregivers, and healthcare professionals in a team-based care model.
Additionally, insights gained from the
Comprehensive Medication Management in Primary Care study, which validated the steps outlined in the PPCP, further guided the revision process.
Next Steps for Implementation
To support the transition to the 2025 PPCP, JCPP is launching an
Implementation Task Force. This group will focus on developing effective messaging and communication strategies to help pharmacists interpret, understand, and implement the updated PPCP effectively. Resources will also be created to facilitate integration into diverse pharmacy practice environments, as well as in education and training programs.
The 2025 Pharmacists' Patient Care Process is now available for access at
JCPP Portal. Pharmacists are encouraged to utilize these guidelines to enhance their practice and contribute significantly to patient care.

In conclusion, the 2025 PPCP is more than just a revision; it is a forward-thinking guideline that empowers pharmacists to integrate their expertise into patient-centered care delivery, thereby optimizing medication outcomes and enhancing overall health in the communities they serve.
