Latest Phase III Trial: Tam-Peli Offers Hope for Small-Cell Lung Cancer Patients

Significant Advances in Cancer Treatment: Tam-Peli



In the ongoing fight against relapsed small-cell lung cancer (SCLC), groundbreaking results have emerged from the Phase III TAISHAN-302 trial, presenting tambotatug pelitecan (Tam-Peli) as a promising second-line treatment option. This study, which captured substantial attention at the International Association for the Study of Lung Cancer's 2026 World Conference, showcased Tam-Peli's remarkable efficacy in improving patient survival rates when compared to the conventional treatment, topotecan.

Study Overview



The TAISHAN-302 trial involved a rigorous evaluation of 451 patients diagnosed with relapsed SCLC, a condition notorious for its aggressive nature and limited treatment alternatives following initial therapies. Participants were randomly assigned to receive either Tam-Peli or topotecan, with the former administered intravenously at a dosage of 2.0 mg/kg at the start of each three-week cycle. The overarching aim of the trial was to determine the overall survival, alongside other key markers, such as progression-free survival and objective response rate.

Promising Results



Recent data revealed that after a median follow-up period of 9.4 months, those treated with Tam-Peli demonstrated a median overall survival of 13.3 months, significantly surpassing the 9.4 months recorded for patients taking topotecan. Notably, Tam-Peli was associated with a striking 54% reduction in the risk of death (HR 0.46; 95% CI 0.35–0.62; p<0.0001). This survival advantage was consistently observed across various patient subgroups, including those with brain and liver metastases.

Additionally, Tam-Peli excelled in enhancing progression-free survival, achieving a median of 7.4 months versus just 2.8 months for topotecan—indicating a 71% decrease in the likelihood of cancer progression or death (HR 0.29; 95% CI 0.23–0.37; p<0.0001). Moreover, the objective response rate, which measures the percentage of patients whose cancer shrinks or disappears after treatment, was remarkably higher with Tam-Peli (59.1% versus 9.7% for topotecan, p<0.0001).

Safety Profile



One of the trial's pivotal benefits was the manageable safety profile of Tam-Peli, which aligned with prior expectations and showed no new concerning safety issues. The rate of treatment-related adverse events classified as Grade 3 or higher was 46.4% for the Tam-Peli recipients compared to 74.7% in the topotecan group, with no recorded treatment-related fatalities among Tam-Peli users. Although instances of treatment-emergent interstitial lung disease were higher in the Tam-Peli cohort (4.9% vs. 1.4%), the rates of severe events were equal across both treatments, highlighting a favorable risk-benefit ratio for Tam-Peli.

Professor Li Zhang from Sun Yat-sen University Cancer Center emphasized the significance of these findings, stating, "TAISHAN-302 stands out as one of the first Phase III studies of an antibody-drug conjugate to yield statistically significant and clinically meaningful improvements in overall survival, progression-free survival, and objective response rate over topotecan in relapsed SCLC. With a generally favorable safety profile, these results position Tam-Peli as a potential new standard-of-care treatment for patients in need of effective second-line therapies."
