OncoBayes Advances OB-001 to Overcome ADC Resistance and Enhance TKI Efficacy

OncoBayes Makes Significant Progress with OB-001



OncoBayes, a clinical-stage pharmaceutical company, recently declared the successful conclusion of preclinical trials for its exciting drug candidate, OB-001. This breakthrough addresses critical challenges in oncology, particularly those associated with antibody-drug conjugate (ADC) resistance and brain penetration of tyrosine kinase inhibitors (TKIs).

OB-001 is notable for its selective inhibition of two crucial efflux pumps, P-glycoprotein (P-gp) and Breast Cancer Resistance Protein (BCRP). These pumps are found both on the blood-brain barrier and within tumor cells, which are notorious for hindering drug effectiveness. As a result, OB-001 is being formulated as a once-daily oral tablet designed to enhance the efficacy of existing oncology therapies.

In recent studies conducted on perfused-brain mice, OB-001 pretreatment led to a remarkable increase in the brain exposure of several leading TKIs while leaving systemic (plasma) exposure relatively unchanged. Notably, data presented at the 2026 AACR conference indicated that OB-001 could increase brain levels of osimertinib by nearly 400% without significant systemic alterations. Such an enhancement promises to lead to substantial improvements in progression-free survival among patients with EGFR-mutated non-small cell lung cancer (NSCLC) and may also benefit those suffering from breast cancer with brain metastases.

The same efflux pumps targeted by OB-001 have been recognized as significant contributors to ADC resistance. Overexpression of these pumps results in diminished drug concentrations within tumors, posing a major hurdle for effective treatment. Preclinical evaluations utilizing a bidirectional transporter assay demonstrated OB-001's capacity to reduce efflux for several prominent payloads, including SN-38 (associated with Trodelvy®), DM4 (linked to Elahere®), and DXd (related to Enhertu®).

With the groundwork laid, OncoBayes is preparing to embark on a clinical trial program aimed at generating proof-of-concept (POC) data for OB-001 in conjunction with osimertinib. The company also plans to assess its potential for re-sensitizing resistant tumors to T-DXd therapies.

Jim Millen, the CEO of OncoBayes, expressed optimism about this breakthrough, stating, “This is a critical milestone for our team, and we are now well positioned to initiate our clinical plans with OB-001. This drug is on track to become a first-in-class therapy with broad potential as a straightforward oral addition to numerous blockbuster oncology treatments.” Millen emphasized the potential of OB-001 to redefine treatment strategies, summarizing it as a “pipeline in a product opportunity.”
