Cumberland Pharmaceuticals Moves Forward with Ifetroban NDA Filing for DMD Cardiomyopathy Treatment
Cumberland Pharmaceuticals to File New Drug Application for Ifetroban
Cumberland Pharmaceuticals Inc. has recently announced its intent to file a New Drug Application (NDA) with the U.S. Food and Drug Administration (FDA) for its product candidate, Ifetroban. This biopharmaceutical company focuses on innovations to treat rare diseases, and Ifetroban is being developed to address cardiomyopathy linked to Duchenne muscular dystrophy (DMD).
The Nashville-based company has taken several steps leading up to this decision. Cumberland had requested a pre-NDA meeting with the FDA, which took place this fall. During this crucial meeting, they discussed the necessary format, content, and analysis required for the submission. Cumberland has proposed the FIGHT DMD™ trial as a pivotal study that will feature comparison data from two natural history databases, further substantiating their application for the drug’s approval.
The FDA granted Cumberland a rolling review of the NDA, which is a significant development as it allows the company to submit completed sections of the application for review prior to filing the entire document. This decision to proceed with the NDA follows an End-of-Phase 2 meeting with the FDA in September 2025 and a subsequent Type C meeting in January 2026. During these discussions, the FDA suggested conducting a Phase 3 trial while raising several issues concerning the results from the Phase 2 study, including questions about the suitability of the left ventricular ejection fraction (LVEF) endpoint and the corresponding statistical analysis plan.
In response, Cumberland shared additional data that supports their efficacy claims, alongside natural history data from a second registry focused on DMD. Dr. Jonathan Soslow from Vanderbilt University Medical Center expressed optimism about the findings from their natural history study, highlighting that heart function notably declines in boys and young men with DMD, and a lower ejection fraction is closely associated with increased mortality rates.
A.J. Kazimi, CEO of Cumberland, emphasized the urgency of submitting the NDA, noting the immediate needs of patients suffering from DMD, particularly concerning their heart health. With favorable findings regarding safety and a promising dose-response observed in the FIGHT DMD™ trial, the company is eager to provide a new treatment option for DMD patients, especially since no current treatments specifically focus on their cardiomyopathy.
The planned NDA submission for Traditional Approval will include data from the FIGHT DMD™ clinical study and its ongoing open-label extension, as well as data from two natural history databases. The FDA has raised concerns that indicate the necessity for substantial evidence of the drug’s efficacy, requesting additional analyses and information, all of which Cumberland is prepared to incorporate into their submission.
Pat Furlong, the Founding President of Parent Project Muscular Dystrophy (PPMD), underscored the community’s concerns regarding heart disease, stating that it remains a top priority for families with DMD patients. The efficacy results from the 12-month FIGHT DMD™ trial demonstrated a remarkable 5.4% absolute improvement in LVEF (p=0.002) when comparing high-dose Ifetroban treatment with a control group comprising placebo-treated patients alongside matched natural history patients. Moreover, there were also significant reductions in markers of cardiac damage among the high-dose group.
The trial showcased that every patient completing the 12-month study expressed interest in continuing with the open-label extension, underlining the treatment's relevance and potential benefits. Ongoing analyses of long-term cardiac MRI data from this extension will further support the NDA filing.
The design of the FIGHT DMD™ trial featured cardiac MRI as a critical method to assess heart function, capable of detecting subtle changes well in advance of heart failure symptoms. Dr. Larry Markham, a pediatric cardiology expert, underscored the importance of this research, emphasizing that cardiomyopathy has become a leading cause of mortality in DMD, and currently, there are no approved treatment options tailored for this specific condition.
As the FDA progresses with its evaluations, Ifetroban's potential benefits have garnered attention, especially given that there are limited targeted therapies for DMD's cardiac complications. The drug has received Orphan Drug Designation, Rare Pediatric Disease Designation, and Fast Track Designation from the FDA, highlighting the vital medical need it seeks to address.
Cumberland has successfully secured a robust portfolio of patents safeguarding Ifetroban for DMD heart disease indications, with the FDA conditionally accepting DYSCORBAN® as its proprietary name. The company is also actively pursuing Ifetroban for additional indications, having recently completed a Phase 2 study for patients with systemic sclerosis while also studying its effects in preventing metastasis in high-risk solid tumors.
In summary, the forthcoming NDA submission marks a crucial step for Cumberland Pharmaceuticals as it aims to transform the landscape of treatment for DMD-associated cardiomyopathy. With the community eagerly anticipating potential advances in therapy options, Cumberland’s efforts illustrate a commitment to meeting significant unmet medical needs for patients and their families dealing with DMD.
