Eli Lilly and Company recently made headlines with new findings from their TOGETHER-PsO Phase 3b clinical trial. This study focused on the effects of combining Taltz (ixekizumab) and Zepbound (tirzepatide) in treating adults battling moderate-to-severe plaque psoriasis alongside obesity or overweight conditions. Conducted in Las Vegas and covering a significant patient population, these exploratory data suggest a noteworthy immunometabolic relationship between psoriasis and obesity, shedding light on interconnected health challenges for these patients.

Findings from the Trial



At the heart of this analysis was the prespecified examination of circulating proteins and gene expressions in patients' blood. It revealed that the synergetic use of Taltz and Zepbound resulted in broader changes to biomarkers associated with psoriasis and metabolic health as early as Week 12 of the treatment, compared to Taltz monotherapy. The data showed a significant increase in the number of differentially expressed proteins and genes, with 482 proteins and 467 genes affected in the combination group, in contrast to just 140 proteins and 16 genes in the Taltz-only group at Week 36.

Moreover, the combination resulted in more pronounced reductions in inflammatory immune activity, including notable changes in neutrophils, crucial immune cells involved in inflammation. Dr. James G. Krueger from The Rockefeller University commented on the groundbreaking nature of these findings, noting how they reveal critical differences in inflammatory pathways when Taltz and Zepbound are used together for these health conditions. This intersection of immune and metabolic health opens the door for future research aimed at providing multifaceted care for psoriasis and obesity patients.

Clinical Outcomes and Patient Management



Earlier reports from the TOGETHER-PsO trial had already highlighted that patients receiving Taltz and Zepbound together experienced superior skin clearance and a clinically significant reduction in weight compared to those on Taltz alone. Promisingly, these improvements sustained or even intensified as the study progressed to Week 52. The safety profile remained consistent with the individual medications, showing that the combined approach is well-tolerated.

As part of the study design, 274 participants were divided into two groups: one receiving Taltz only, and the other a combination of Taltz with Zepbound. Both groups were encouraged to adopt a healthier lifestyle, involving a reduced-calorie diet and increased physical activity. The primary endpoint focused on determining how many participants achieved a PASI score of 100 and a 10% reduction in weight by Week 36.

Taltz is a monoclonal antibody targeting the IL-17A cytokine, essential for inflammatory and immune responses. This medication is used to treat various conditions, including active psoriatic arthritis and moderate-to-severe plaque psoriasis in adults.
