Novo Nordisk's Wegovy® Shows Promising Results in Liver Health Improvement for Non-Obese Patients

Advancements in Liver Health: Wegovy® 2.4 mg Shows Effectiveness in Treating Non-Obese Patients with MASH

At The Liver Meeting® 2026, Novo Nordisk unveiled intriguing results from a post hoc analysis of its phase 3 ESSENCE trial, focusing on the efficacy of Wegovy® (semaglutide 2.4 mg) in adults grappling with metabolic dysfunction-associated steatohepatitis (MASH) and presenting with liver scarring. This analysis is particularly significant as it highlights improvements in liver health among a demographic often overlooked – individuals with a body mass index (BMI) of less than 27 kg/m².

Key Findings

This analysis surveyed a subset of 106 adults who were randomly selected and compared outcomes between those treated with Wegovy® and a placebo. The findings revealed a notable enhancement in liver-related health markers after 72 weeks of treatment. The improvements included reductions in liver enzymes and other vital indicators of liver injury and fibrosis, as detailed below:
  • - Alanine aminotransferase (ALT) reduction: Estimated treatment ratio (ETR) 0.61 (95% CI 0.48, 0.79)
  • - Aspartate aminotransferase (AST) reduction: ETR 0.67 (95% CI 0.56, 0.80)
  • - Disease Activity (FAST score): Estimated treatment difference (ETD) -0.13 (95% CI -0.26, -0.01)
  • - Enhanced Liver Fibrosis (ELF) score reduction: ETD -0.45 (95% CI -0.73, -0.17)
  • - PRO-C3 marker improvement: ETR 0.82 (95% CI 0.71, 0.95)
  • - VCTE-LSM (liver stiffness measurement): ETR 0.88 (95% CI 0.73, 1.06)

These results suggest that Wegovy® can effectively address liver injury and fibrosis in individuals typically seen as low-risk due to their weight.

The Importance of These Findings

Dr. Michelle Skinner Moore, Vice President of Medical Affairs at Novo, pointed out the disease's often unnoticed nature: "MASH can affect people of any body type. Its symptoms are often silent and untreated cases can lead to severe health complications." This comprehensive analysis indeed shines a spotlight on a critical issue, emphasizing that MASH often coexists with individuals of normal weight, thereby challenging the conventional understanding that links the disease primarily with obesity.

Professor Jörn Schattenberg, a notable figure in gastroenterology at Saarland University Medical Center, reiterated the message embodied within the findings: body size alone should not dictate the seriousness with which we regard MASH. He stated, "This analysis suggests a pattern across measures of liver injury, disease activity, and fibrosis, reinforcing the need for broader attention to this condition."

ESSENCE Trial Background

The ESSENCE trial is formatted into two parts. The initial phase consists of evaluating the liver histology changes in 1,197 participants after 240 weeks of treatment, while the ongoing second phase aims to assess the risk reduction of liver-related clinical events among adults suffering from moderate-to-advanced fibrosis (F2 or F3). The promising results shared by Novo Nordisk from this trial represent a pivotal step in better understanding and managing MASH.

What's Next?

With over 250 million people afflicted by MASH globally, the urgency for expanded treatment options is clear. The burgeoning data around Wegovy® potentially being effective in a wider patient demographic offers hope not just for individuals with obesity but also for non-obese patients grappling with this serious condition. As Novo continues its research, the industry's focus will be on validating these findings through confirmatory trials to solidify the clinical benefits of Wegovy® in managing MASH.

Ultimately, the findings reinforce modern healthcare's direction towards personalized treatment avenues, acknowledging that metabolic dysfunction is not solely restricted to those categorized as obese. As health professionals strive to diagnose and manage MASH comprehensively, the emergence of such studies enhances the scope for potentially life-saving interventions, ensuring that everyone affected by this silent affliction receives the care they need.
