Recent research conducted by the Sunstar Group, in collaboration with Jichi Medical University, has unveiled significant connections between oral health and metabolic dysfunction-associated steatotic liver disease (MASLD). Background of the Study


As lifestyle changes and Western dietary influences have led to increased obesity in Japan, it is estimated that over 20 million people suffer from fatty liver disease. In light of new classification standards from international forums, what was previously known as non-alcoholic fatty liver disease (NAFLD) will soon be rebranded as MASLD starting in August 2024. Given that MASLD is closely linked with lifestyle diseases and metabolic disorders, it poses a high progression risk toward cirrhosis or liver cancer, making its prevention essential.

Previous studies have established a relationship between NAFLD and periodontal disease; however, there has been a notable lack of research exploring the correlation with oral functionality. The Sunstar Group's study aimed to bridge this gap by analyzing the oral health and functions of MASLD patients compared to healthy individuals.

Research Methodology


The research involved a sample of 19 MASLD patients (average age 54.1 years) from Jichi Medical University and 26 healthy participants (average age 48.4 years) from the university’s health screening center and a manufacturing firm. Participants were evaluated using ultrasound examinations and standardized questionnaires to measure the impact of oral health on their quality of life (QOL). The study employed the General Oral Health Assessment Index (GOHAI) for evaluation alongside meticulous oral exams conducted by dental professionals to assess conditions like plaque, periodontal disease, cavities, and oral functionality criteria.

1. Impact on Quality of Life

The analysis demonstrated that MASLD patients experienced more profound impacts on QOL due to oral issues compared to their healthy counterparts. Differences were particularly marked in activities involving eating and speaking.

2. Correlation Between MASLD and Oral Function

The comparison between the oral conditions of MASLD patients and healthy individuals revealed statistically significant differences in oral hygiene, the prevalence of cavities, and periodontal health. Despite no significant variance in chewing ability or tongue strength, noteworthy differences arose in the number of functioning teeth and salivary output. Adjusted analyses maintained these relationships, displaying independent associations between MASLD and both periodontal disease and the number of functional teeth.

Conclusion


The research unveiled that individuals with MASLD face significant challenges concerning oral health, which directly influence their daily lives. The findings reinforce the notion that maintaining oral health is crucial in reducing MASLD risks. It also suggests that beyond periodontal disease, the number of teeth capable of effective chewing plays a fundamental role in mitigating risks associated with MASLD. As MASLD patients often struggle with maintaining oral cleanliness, oral care is paramount, paving the way for potential collaborative healthcare approaches that integrate both oral and physical health.

Dr. Kouichi Miura from Jichi Medical University's Department of Internal Medicine emphasized the increasing recognition of oral health's impact on various diseases. He noted that although previous studies have linked periodontal disease to fatty liver, the implications for oral functionality and quality of life have not been adequately investigated.
