Aetna Expands Clinical Collaboration Program



Aetna®, a prominent health care company under CVS Health®, has announced its plan to extend the Aetna Clinical Collaboration (ACC) program to ten hospitals by the end of 2025. This initiative aims to significantly enhance support for hospitals while providing personalized member care for those enrolled in Medicare Advantage plans.

The ACC program focuses on creating a collaborative environment where Aetna nurses work hand in hand with hospital staff. This teamwork is designed to ensure that Medicare Advantage members receive the necessary support post-discharge, helping them transition smoothly from the hospital to their homes or to skilled nursing facilities.

A concerning statistic highlights the urgency of this initiative: nearly 20% of hospitalized Medicare-insured patients find themselves rehospitalized within 30 days of discharge. The ACC program is envisioned to tackle this challenge by decreasing the rates of 30-day readmissions and emergency department visits, as well as alleviating the administrative burden faced by hospitals.
