In an enlightening message, prominent cardiologist Dr. Andrew P. Ambrosy has brought attention to the significant advantages of using GLP-1 and dual GIP-GLP-1 inhibitors for those at risk of cardiovascular disease. These medications, including semaglutide (commercially known as Wegovy) and tirzepatide (Zepbound), have shown promising results in clinical trials yet remain underutilized.
Dr. Ambrosy, who serves as a physician-scientist at Kaiser Permanente Northern California, pointed out that although the FDA approved semaglutide in March 2024 to help decrease the risk of cardiovascular-related complications such as heart attack and stroke, there is a troubling gap in the number of patients adopting this life-saving treatment.
"Despite the evident health benefits that these medications offer in reducing the risk of heart attacks and strokes, a mere 5% of eligible patients are currently taking them," he stated. This indicates a glaring opportunity for improvement in patient care, as he believes that many lives could be saved with increased use of these drugs.
GLP-1 receptor agonists function by mimicking a natural gut hormone that regulates appetite and blood sugar. The dual agonists work on two hormonal pathways—GIP and GLP-1—leading to remarkable effects on weight management. Furthermore, some patients participating in clinical trials experienced over 20% weight loss, a feat typically only achieved through bariatric surgery.
A recent analysis involving 17,000 adults facing heart disease who are overweight or obese found that semaglutide led to a 20% reduction in the risk of heart attack, stroke, or death from cardiovascular complications, often showing benefits within mere months of treatment onset, well before significant weight loss occurred.
Extensive research has also documented how both semaglutide and tirzepatide can enhance heart health and alleviate symptoms of heart failure, along with improving patients' overall quality of life. In particular, patients noted improved capabilities in walking and exercising, bolstering the potential for a higher standard of living.
Dr. Ambrosy noted that many heart disease patients also face additional health challenges such as kidney issues and the risk of developing type 2 diabetes. An enlightening trial involving over 3,500 adults with this condition and chronic kidney disease revealed that semaglutide not only supports heart health but also helps to decelerate the decline of kidney functions, thereby reducing the risk of kidney-related failures and deaths.
Highlighting the interconnectedness of heart, kidney, and metabolic health, Dr. Ambrosy remarked, "The systems work together, indicating multiple health benefits from utilizing these types of drugs."
Moreover, the current options for GLP-1 inhibitors come in daily pills or weekly injections, with promising research underway for once-monthly injections, as ongoing cardiovascular clinical trials explore this innovative approach. Such advancements would greatly benefit patients who struggle with daily medication adherence or dislike needles.
Despite tangible evidence that these drugs can mitigate cardiovascular risks, Dr. Ambrosy expressed concern over disparities in access to this potentially life-saving medication. He pointed out that studies consistently reveal that Black and Hispanic adults, as well as those enrolled in Medicaid or residing in rural, low-income areas, are notably less likely to receive prescriptions for these medications.
"It is invaluable to ensure availability for all patients to access these treatments, as closing this gap is just as critical as the groundbreaking science that supports their use," he concluded, emphasizing the responsibility of healthcare providers to advocate for vulnerable populations.
