Marengo Therapeutics Secures Second FDA Fast Track Designation for Invikafusp Alfa, Broadening Its Scope to High Tumor Mutational Burden Solid Tumors
Marengo Therapeutics Expands FDA Approval Pathway for Invikafusp Alfa
On October 15, 2026, Marengo Therapeutics Inc., a clinical-stage biotechnology firm based in Cambridge, MA, announced a significant advancement in its immuno-oncology portfolio. The U.S. Food and Drug Administration (FDA) has granted Invikafusp alfa, Marengo's lead candidate, a second Fast Track Designation. This new designation is pivotal, allowing for the treatment of unresectable, locally advanced, or metastatic solid tumors characterized by a high tumor mutational burden (TMB-H) that have progressed after prior treatments.
This expanded designation moves beyond its initial scope in January 2025, which was specifically focused on TMB-H colorectal cancer. With this new approval, Invikafusp alfa can now be utilized for a broader range of solid tumors, encompassing various types including colorectal, gastric, breast, urothelial, endometrial, gastroesophageal, head and neck, and lung cancers. The evidence supporting this expansion includes objective responses gathered from patients across multiple tumor types as indicated in the ongoing Phase 1/2 STARt-001 study.
Dr. Eric Rubin, a prominent member of Marengo's Scientific Advisory Board and a former Senior Vice President at Merck, emphasized the innovative approach of Invikafusp alfa. Unlike conventional therapies that release checkpoints on exhausted T cells, Invikafusp alfa actively enhances the presence of new T cells designed to recognize tumor antigens. This offers hope for patients whose tumors are non-responsive or who have relapsed after PD-1 blockade treatment. As Dr. Rubin stated, “Invikafusp alfa takes a fundamentally different approach” which renders it a promising candidate for targeting these challenging tumor types, irrespective of their origin.
The Fast Track Designation fosters a productive environment for drug development, enhancing communication with the FDA and optimizing review processes for medications addressing serious conditions with unmet needs. New Phase 2 data, to be presented at the European Society for Medical Oncology (ESMO) Congress on October 23, 2026, will further confirm the efficacy of Invikafusp alfa in this patient population.
Dr. Ke Liu, Marengo’s Chief Development Officer, expressed confidence in Invikafusp alfa’s capacity to become a foundational treatment option in immuno-oncology. With positive monotherapy activity exhibited in heavily pre-treated patients resistant to checkpoint inhibitors, the company is now exploring its use in conjunction with standard-of-care therapies, including antibody-drug conjugates, to potentially expand its applicability in earlier treatment lines. “Our vision is to position Invikafusp alfa as a next-generation, tissue-agnostic backbone in immunotherapy,” said Dr. Liu.
About Invikafusp Alfa and Its Clinical Journey
Invikafusp alfa (STAR0602) is a groundbreaking bispecific dual T-cell agonist. It uniquely activates a T-cell subset prevalent in numerous cancers by merging non-clonal TCR activation with a co-stimulatory signal, thereby promoting a diverse and robust T-cell response against tumors. The STARt-001 clinical trial, which is currently being conducted, assesses Invikafusp alfa's efficacy in patients with advanced solid tumors resistant to PD-1-targeted therapies. The study intentionally selects biomarker-enriched cohorts to maximize identification of patients who are likely to derive benefit from the treatment.
Despite its promising potential, it's important to note that Invikafusp alfa is still under investigation and has not received approval from the FDA or any other health authority.
