Vivant Health Achieves Impressive Ten Million Dollar Milestone in Shared Savings for Medicare Patient Care

Vivant Health ACO Reports Over Ten Million in Shared Savings

Vivant Health has recently announced a significant achievement: its Accountable Care Organization (Vivant Health ACO) has surpassed ten million dollars in total shared savings over the last three performance years. This includes an impressive $1,960,425 saved during the Performance Year 2024. These results highlight the organization's commitment to delivering high-quality care and improving health outcomes for medically complex Medicare populations.

The Vivant Health ACO primarily serves a high-risk Medicare demographic through partnerships with six Federally Qualified Health Centers (FQHCs). These centers collectively manage nearly 68% of ACO beneficiaries, providing holistic care that addresses not just chronic diseases but also the intricate interplay of clinical complexities, behavioral health needs, and the social determinants that affect health outcomes.

Under the guidelines of the Medicare Shared Savings Program, 60% of all shared savings is allocated directly to the participating providers, thus emphasizing their essential role in delivering quality care. Of the total savings above ten million dollars, over six million dollars have been returned to these provider partners over the last three years, which includes $1,176,255 for Performance Year 2024 alone.

Sarath Artham, the CEO of Vivant Health, recognizes the dedication and compassion of the six FQHC partners who collaborate closely with them. He stated, "Their commitment, compassion, and relentless focus on patient needs are the foundation of our results. By reinvesting shared savings directly into our provider partners, we not only enhance community health but also secure ongoing improvements in care quality and outcomes for every patient we serve."

Vivant Health ACO maintains a robust record of shared savings, strategic quality improvement, and health management that caters to high-need populations. As the Centers for Medicare & Medicaid Services (CMS) pushes the value-based care initiative, Vivant Health stands in a unique position to support these goals through its deep-rooted partnerships and commitment to serving communities in need.

The organization actively participates in the Enhanced Track of the Medicare Shared Savings Program, where it consistently demonstrates high performance on national metrics involving preventive care, chronic disease management, and the utilization of hospital resources, especially for underserved Medicare populations.
