Each selected organization will receive a grant of $1 million aimed at improving healthcare access and resources for older adults dealing with chronic conditions related to brain, heart, and behavioral health.   • - EngageWell Independent Provider Association: Operating in New York, NY, EngageWell plans to use the funding to enhance its telemedicine capabilities and expand its Care Your Way initiative, which supports low-income elders of color facing healthcare access challenges.   • - Palm Health Foundation: In Boynton Beach, FL, this foundation aims to create a Healthier Aging Initiative, with a specific focus on supporting caregivers aged over 60.
