Contexture Partners with Colorado STREAM to Enhance Rural Healthcare Coordination and Accessibility

Contexture Collaborates with Colorado STREAM Rural Health Network

Contexture, a nonprofit organization dedicated to improving community health through collaborative information sharing, has recently joined forces with the Colorado STREAM Rural Health Network. This initiative is spearheaded by the Denver Recovery Group (DRG), which has been awarded a substantial grant of $4.9 million from the state’s Rural Health Transformation Program (RHTP). With these resources, the STREAM project is set to forge stronger connections among opioid treatment programs, community pharmacies, and healthcare teams across the rural and mountainous regions of western Colorado.

The Need for Enhanced Care in Rural Areas

Rural healthcare access has always posed challenges, including long travel distances, lack of transportation, and fragmented services. By integrating health information exchange (HIE) technology, Contexture aims to alleviate these hurdles through improved communication and coordinated care efforts. The goal is straightforward: to make opioid treatment and overall healthcare services easier to access for rural populations who face significant barriers.

The STREAM initiative will establish a comprehensive network that connects five DRG-operated opioid treatment locations with nine community pharmacies. This hub-and-spoke operational model is designed to embrace a wide array of services, including treatment, telehealth consultations, referrals for services, and patient navigation. By providing these resources closer to where people live, STREAM aims to expedite access to vital services and improve overall health outcomes in the community.

Strengthening Healthcare Networks

As a partner in this project, Contexture will assess interoperability needs and offer workflow planning, thus ensuring a seamless exchange of information among participating organizations. Through their innovative PatientCare 360® Portal, healthcare providers across the network will gain access to essential patient information needed for effective care coordination and referral management.

Ideally, the implementation of the STREAM model will serve as a prototype for future rural health initiatives. Wireless connectivity and secure data handling will enhance patient identification and care coordination significantly. Both consent and privacy will be prioritized, especially given the sensitive nature of substance use disorder records, which are subject to strict confidentiality laws (42 CFR Part 2).

Jackie Sievers, Chief Growth Officer at Contexture, expressed her enthusiasm for the collaboration. "STREAM combines the capabilities of rural providers and community pharmacies to overcome access challenges. We are thrilled to support secure information sharing and help healthcare teams develop effective workflows tailored to community needs." Through this initiative, Contexture hopes to not only improve immediate healthcare access but also contribute valuable insights that could inform future efforts to advance rural health.

Collaborative Efforts and Future Goals

The STREAM project benefits from a collaborative framework that includes numerous stakeholders, such as the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences, CPESN Colorado, Colorado Pharmacists Society, and additional partners committed to mutual goals. This cooperative approach enhances the sharing of expertise spanning clinical, operational, and informational domains, ensuring a well-rounded initiative that can successfully address the complexities of rural health care.

The overall investment in Colorado's RHTP, which totals over $169 million, signals a significant commitment from the state to improve the healthcare landscape in its most underserved areas. With Contexture and its partners at the helm, the future of rural healthcare in Colorado looks brighter, aiming to provide patients with the care they need, where they need it most.

As this initiative unfolds, further evaluations and lessons learned will undoubtedly create pathways for innovation that can be expanded in future RHTP applications. Contexture's work with the STREAM Rural Health Network stands as a testament to the potential for meaningful change in healthcare access and coordination in rural communities across the state.
