To tackle this problem, ACLM has released the Lifestyle Medicine Whole Person Health Index (LMWPHI), a point-of-care assessment tool aimed at streamlining the integration of holistic elements into everyday clinical practices. Currently embedded within Epic Foundation systems nationwide, this innovative tool will soon relaunch to facilitate broader access and effectiveness across various healthcare settings.
What is the LMWPHI?
The LMWPHI is designed as a brief, patient-reported assessment that can be completed prior to or during a patient visit. It focuses on the six essential pillars of lifestyle medicine, which have strong associations with chronic disease risk:
