Analyzing Customer Harassment in Healthcare: Urgent Need for Organizational Intervention

Understanding Customer Harassment in Healthcare: Crucial Insights from a Recent Report

Kazuhiro Hamakawa, CEO of Wecan Co., a Tokyo-based company, recently launched the Customer Harassment White Paper 2025 Edition, revealing critical concerns regarding customer harassment (CSD) in the healthcare sector. The report compiled data from approximately 1,000 harassment consultations, emphasizing that around 70% of cases demand urgent organizational action to address escalating issues.

Key Findings from the Report

The report analyzed 995 cases of harassment reported by healthcare and welfare workers alongside 1,306 consultations from patients and families, totaling 2,301 records. While the number reflects substantial awareness of the problem, it indicates that the real situation may be much direr. Starting October 1, 2026, organizations in any sector, including healthcare and elder care, will face a legal obligation to proactively manage customer harassment, making the findings of this report timely and crucial.

1. Urgent Need for Intervention: The categorization of the 995 consultations into three risk levels revealed alarming trends. About 68% (677 incidents) fell under risk level three, which clearly indicates that organizational intervention is non-negotiable. A further 24% were classified as risk level two, portraying high stress levels being experienced in these environments.

2. Lack of Awareness of Early Warning Signs: Traditionally, concepts like Heinrich’s Law suggest that early warning signs of danger are more numerous than severe cases. However, this report revealed the opposite: only 8% of cases showed early warning signs, suggesting that when incidents do emerge, they are often far along in severity, and warnings have gone unrecognized until it is too late.

3. Estimates of Continuing Incidents: The 677 severe cases correspond to a higher volume of unreported or unresolved incidents, with estimates suggesting that actual cases being addressed yearly amount to around 10,000 incidents. This stark statistic highlights the significant gap between reported and unreported cases.

4. Average Consultation Time Indicates Severity: On average, consultations lasted over 27 minutes, with nearly 40% exceeding half an hour. Longer consultations imply escalated situations, where substantial time has already been consumed before an issue is formally acknowledged and documented.

5. Profile of Perpetrators: Delving into the nature of the harassment, it was found that about half of the aggressors were the customers themselves, while the family members of customers made up the other half. The data indicates that verbal abuse (42.5%) and unreasonable demands (30.5%) overwhelmingly characterized the incidents reported.

6. Underlying Structures Need Addressing: The report goes beyond simply categorizing incidents; it explores the deeper structural issues leading to harassment behaviors. For example, a staggering 34% of incidents related to contractual boundary violations, which underscores that merely addressing the immediate behaviors without resolving these structural issues will likely lead to reoccurrence.
