This accolade, awarded by the Health Resources and Services Administration (HRSA), places Care Resource among the top health centers in the nation for its clinical quality performance in 2025. This achievement highlights the center's unwavering commitment to advancing community health and provides a robust framework for excellent health services.

Numerous Recognitions



At the forefront of this recognition, Care Resource received multiple Community Health Quality Recognition (CHQR) badges, attesting to its exceptional performance across various health domains. Notably, the health center has been awarded National Quality Leader (NQL) badges in critical areas such as Behavioral Health, Heart Health, and Diabetes Health. These NQL badges affirm Care Resource's position as a leader in health care, where it continually meets or surpasses national benchmarks in essential care sectors, including depression screening, cardiovascular disease prevention, and diabetes management.

In addition to NQL badges, Care Resource retains its Gold Health Center Quality Leader (HCQL) badge, marking its place within the top 10% of health centers nationwide based on overall clinical quality performance. The center's recognition in areas such as Advancing Health Information Technology for Quality reflects its progress in using innovative telehealth services, patient engagement tools, and data-driven care coordination. Not only has Care Resource excelled in avoiding preventable health issues, but its focus on preventive health practices, including tobacco cessation programs and body mass index (BMI) screening, has also shown impressive results that exceed state and national benchmarks.

A Commitment to Community Health



Dr. Steven Santiago, Chief Medical Officer at Care Resource, expressed pride in the recognition, stating, "Being recognized with the 2025 Health Center Quality Leader Gold badge and multiple National Quality Leader designations is a proud achievement for Care Resource. Over the past year, Care Resource facilitated over 130,000 patient service visits and served over 26,000 unique clients, with an impressive 53% of these patients lacking health insurance. The center showcased its dedication to improving lives by dispensing more than 30,000 prescriptions, providing over 529,000 meals through its Food for Life Network food pantry, and achieving a remarkable 97% linkage rate for HIV care within 30 days of diagnosis—surpassing national averages significantly.

Innovative initiatives such as Care Resource's Remote Patient Monitoring (RPM) program have expanded access to healthcare for patients dealing with chronic conditions, such as hypertension and diabetes, further demonstrating the center's commitment to improving health outcomes in the community. Additionally, Care Resource's mobile outreach team, affectionately known as The SPOT, distributed nearly 10,000 doses of Narcan as part of its commitment to combating the opioid crisis. With housing assistance programs through HOPWA and other initiatives, they have provided support to over 1,400 individuals and families in need of housing.
