Overview of the Tele-Dentistry Programme


In a significant move to tackle children's dental health issues, the National University Health System (NUHS) has launched a new tele-dentistry initiative. This program, named the HEAlth and Development SUpport in Preschool Partnerships (HEADS-UPP), is designed especially for families at risk of dental caries. Traditionally, dental visits have often been reactive, often only occurring when a child shows visible problems or pain. However, this new program aims to change the narrative by empowering parents to take proactive steps for their children's oral health.

Addressing the Dental Health Gap


Research shows alarming dental health disparities among children from lower-income families, with studies indicating that childhood caries prevalence rises sharply with age. One significant study revealed that as many as 42.9% of three-year-olds are at risk, a stark increase from 17.8% in two-year-olds. Furthermore, access to dental care varies greatly based on household income, with many children from less affluent families failing to visit a dentist due to barriers such as cost and access to care.

How the Programme Works


The HEADS-UPP tele-dentistry programme combines behavioral risk assessments with advanced image technology. After enrolling, children aged 18 months to six years from selected preschools receive in-person assessments where trained nurses capture intraoral images of their teeth. Alongside this, parents complete a questionnaire that adapts the Caries Risk Assessment tool, enabling customized feedback considering the child's oral hygiene habits, dietary intake, and family history. This dual approach ensures that each child's unique needs are addressed.

A pediatric dentist then reviews these assessments, producing a comprehensive dental report for each child. This personalized report includes visuals of areas needing attention and actionable recommendations for follow-up care.

Empowering Families


What sets this initiative apart is not just the technological implementation but the emphasis on parental guidance. The dental reports elucidate the next steps for parents without overwhelming them, encouraging questions and conversations about their child's dental health. Approximately six to nine months later, follow-up questionnaires allow the team to gauge the effectiveness of the program and the parents' adherence to recommendations.

Results and Positive Impacts


Initial results from the program are encouraging. A staggering 93.3% of children were identified as moderate to high risk for dental caries at the onset. However, through the tele-dentistry program, 57.5% of those recommended for specialist follow-up received the necessary treatment, showcasing a significant improvement in parental follow-through when compared to previous studies.

Moreover, many parents reported strengthening their children's tooth brushing habits and reducing behaviors that contribute to caries risk. The initiative demonstrates not just the health benefit but a growing confidence for families in managing their children's oral care.

Future Directions


The tele-dentistry initiative is not static; the NUHS plans to incorporate artificial intelligence to enhance the efficiency of the assessment processes further. AI will be utilized to automate image analyses and questionnaire evaluations, streamlining the healthcare delivery system and ensuring even more families can access quality dental care.
