Yuvo Health Collaborates with Public Health Solutions



In celebration of Women's History Month, Yuvo Health, a pioneering partner in value-based care, has announced a strategic collaboration with Public Health Solutions (PHS), one of New York City’s preeminent public health non-profits. This partnership aims to address crucial health and social needs by conducting a targeted campaign to complete 40,000 health-related social needs screenings across their collective network of Federally Qualified Health Centers (FQHCs) in New York City by June 30, 2026.

This initiative aims to uncover underlying issues that New Yorkers face, such as food insecurity, housing instability, and transportation obstacles. By identifying these challenges, Yuvo Health and PHS will work together to connect affected individuals with community-based support services tailored to meet these pressing needs.

Janel Sia, Chief Strategy Officer and co-founder of Yuvo Health, emphasized the importance of this initiative in light of concerning statistics: over 25% of children in New York live in poverty. "Women often lead the charge in healthcare decisions for their families. By identifying social needs early, we can drive timely referrals to essential resources and significantly improve health outcomes at a family level," Sia said.

The collaboration aligns with New York State's 1115 Waiver program, which facilitates the integration of healthcare with community-based organizations. Yuvo Health and PHS will assess their impact based on successful patient screenings, referrals, and the effective coordination of services that target social determinants of health.

Zachariah Hennessey, Chief Strategy Officer at Public Health Solutions, remarked, "Sustainability in our programs hinges on demonstrable outcomes. Our longstanding ties within the communities we serve, coupled with Yuvo Health's framework for value-based care, streamline the process by which families—particularly mothers navigating intricate healthcare systems—access appropriate resources."

To kick off this critical effort, Yuvo Health will deploy its Independent Practice Association network to support FQHCs including Joseph P. Addabbo Health Center, Housing Works, and Boriken Neighborhood Health Center. This collaboration aims to minimize administrative challenges while ensuring efficient screening processes.

Hyphen, a technology partner for this initiative, will facilitate data sharing and integration across the involved organizations, supporting real-time insights and operational effectiveness. PHS will oversee referral pathways to ensure patients receive the timely assistance they require.

To date, findings show that approximately 40% of PHS patients screened have reported at least one unmet social need. Connecting these individuals with community services has led to improvements in adherence to medical advice and routine health screenings, showcasing the real-world benefits of these screenings on both personal and communal health outcomes.
