In recent research conducted by Ginza Repro Clinic in Tokyo, it was revealed that approximately 82% of men actively engaged in fertility efforts utilize saunas, a rate significantly surpassing that of other men. This study involved 200 men in their 20s to 30s living in the metropolitan area of Tokyo and its surrounding prefectures, shedding light on thermal exposure’s impact on fertility.

Among the 56 respondents who had either experienced or intended to engage in fertility efforts, a staggering 46 utilized saunas at least once in the past year.

In contrast, only 38% of men who were not pursuing fertility reported sauna usage, representing a significant behavioral gap between these two groups. Furthermore, even among those aware that excessive heat may adversely affect sperm quality — 32% of the fertile respondents — every single individual reported past sauna use.

Despite knowing that heat around the testes can potentially affect sperm health, many did not alter their sauna habits. Notably, around 53% of participants admitted being unaware of how excessive heat could impact sperm quality. When asked why the testes are located outside the body, only 22% could correctly identify that it is because sperm production requires a cooler temperature than that of the human body. A significant majority, therefore, demonstrated a lack of foundational knowledge about testicular biology and its relationship to thermal exposure.

Lifestyle Factors to Consider


Beyond sauna use, a majority of participants reported engaging in various everyday habits that could contribute to higher temperatures in the groin area. For instance, 63% acknowledged habits like sitting with legs crossed for extended periods, wearing tight clothing, or participating in time-consuming activities involving heat exposure, including long baths or using heated seats in vehicles.

During the summer months, 69% of the respondents experienced heat or discomfort in the groin area, with many not taking preventive measures against it. Thirty-eight percent indicated they had no specific strategies in place to manage this heat, showing a gap in actionable awareness regarding their reproductive health.

Recommendations for Healthier Practices


After being informed about the potential adverse effects of heat on sperm health, approximately one-third of the men expressed a desire to modify their sauna practices. They were encouraged to minimize sauna sessions or consider time spent inside the sauna. This is an essential step for those focusing on fertility, as managing heat exposure is pivotal for optimal reproductive health.

As a medical professional, Nagao likened the effects of varicocele to having the testicles perpetually in a sauna, which can impede fertility in men. Men undergoing fertility evaluations might benefit from screening for conditions like varicocele rather than solely attributing fertility issues to lifestyle factors.
