Montefiore's Community Health Worker Institute Aids Over 10,000 Households in the Bronx

A Milestone Achievement for Bronx Communities



The Community Health Worker Institute (CHWI) of Montefiore has made significant strides in enhancing community wellbeing and creating new professional paths since its inception in 2021. More than 10,000 households in the Bronx have benefited from this innovative program, which aims to bridge the gap between healthcare and social services.

Addressing Local Needs



Rooted in community involvement, the CHWI focuses on addressing unmet needs within the Bronx. The Metropolitan area is characterized by distinct health challenges, including housing insecurity and limited access to healthcare, primarily exacerbated by social determinants of health (SDOH). To tackle these issues, Montefiore deployed 34 Community Health Workers (CHWs) across various facilities, making services accessible to residents in places like hospitals and specialized care centers. CHWs play a crucial role in identifying gaps in healthcare access and helping patients navigate these challenges.

In a recent survey conducted among residents, it was found that approximately 14% of patients expressed at least one unmet social need that affected their health. Strikingly, those with multiple needs incurred nearly $1,772 more in healthcare expenses due to avoidable resource use, highlighting the necessity of the program.

Impact on Health Outcomes



The success of the CHWI can be measured not only by the number of households served but also by positive health outcomes. In the past year, a study revealed that CHWs have led to a significant increase in patients reporting improvements in their health conditions, particularly in managing chronic illnesses like asthma and diabetes. After consulting with a CHW, an impressive 94% of patients claimed they made progress in resolving their problems.

The Bronx is not only known for its baseball team, but also for the community spirit that fosters real change,” noted Renee Whiskey-LaLanne, MPH, a vital leader in the initiative. “Through addressing social determinants and enhancing health conditions, we are paving the way for substantial health improvements within the community.”

Expanding Job Opportunities



In addition to improving health, the CHWI is uniquely positioned to foster economic growth through new job creation. Many CHWs are local residents who understand the community’s challenges, creating a bond of trust with patients. As these health workers gain experience, numerous pathways to career advancement become available, allowing them to transition into roles such as supervisors and health trainers, or even to continue their education in fields like nursing and social work.
