Recent research conducted by anesthesiologists at Montefiore Einstein has revealed significant benefits associated with administering fewer intravenous (IV) fluids during surgical procedures. Published in JAMA Network Open, this study set out to investigate the effects of reduced fluid administration, especially during a nationwide shortage of IV fluids that occurred in 2024.

This innovative approach involved a twelve-step intervention to optimize fluid use during surgeries. Strategies included utilizing smaller IV bags, minimizing the number of bags used in operating rooms, and replacing IV hydration with oral hydration wherever feasible. Notably, these adjustments led to a remarkable 75% reduction in IV fluid usage across nearly 27,000 patients undergoing various elective surgeries.

The study meticulously compared the outcomes of these patients with a control group of 12,000 individuals who had undergone surgeries prior to the intervention. The findings were striking; fewer Foley catheter placements and reduced rates of urinary infections were observed, as well as a significant drop in hospital readmissions within 30 days post-surgery. These outcomes suggest that not only is reduced fluid administration safe for patients, but it also contributes to improved surgical results.

Historically, IV fluids are routinely administered during surgeries to ensure appropriate hydration and electrolyte balance. However, there has been ongoing debate regarding the optimal volume of fluids necessary for patients undergoing elective surgeries. The study’s context was particularly timely, as production issues surfaced due to Hurricane Helene in 2024, allowing researchers to conduct a real-world experiment.
