Van Horn is stepping into this key role with a wealth of experience in home-based care and healthcare innovation. Prior to this, he served as the Chief Growth Officer for Optum's Home & Community division, particularly following Optum's acquisition of Landmark Health, a company Van Horn co-founded.
OnDemand Visit is designed to unify both emergency and non-emergency healthcare experiences through inventive programs that promote superior patient outcomes and lower costs. By integrating its extensive Emergency Medical Services (EMS) capabilities, the organization aims to deliver personalized care to patients, ensuring they receive assistance the moment they need it most, whether for emergencies or ongoing wellness.

Van Horn's career includes co-founding Landmark Health, which specialized in offering in-home medical attention to patients with complex chronic health issues, where he held the position of President and Chief Business Officer. His prior tenure as Senior Vice President and General Manager of CareMore Health Plan saw him guiding product-line growth and network management until the company's sale to Anthem.
With deep roots in the EMS and healthcare fields stemming from a family background of nurses, paramedics, and EMS leaders, Van Horn brings a valuable perspective to this role. His introduction to Priority OnDemand came through his work as an advisor on the Operating Council for Consonance Capital Partners, an investment partner with the healthcare company.
Van Horn emphasizes the shift in approach he aims to champion—"Ambulance companies have customarily been reactive to patient needs. We want to extend patient touchpoints beyond traditional emergency responses, adding proactive preventive care."
A graduate of the University of California, San Diego, with a degree in Quantitative Economics and Decision Sciences, he also holds an MBA in Operations from the Anderson School of Management at UCLA.
