'Risetsutel' is an innovative solution that detects when users leave the premises of care facilities in real time and automatically notifies relevant parties via a voice messaging system. By utilizing Bluetooth smart tags that users carry and receivers installed within the facility, it minimizes the burden on staff and does not require specialized construction for installation. This addresses a critical issue faced in nursing care: the often delayed reactions due to structural challenges.

Addressing the Challenge of Premature Exit



Unattended exits, or 'risetsu,' have become a serious challenge in care facilities. Despite the implementation of supervision systems, the risk of unsupervised exits still remains, leading to potential accidents or disappearances. According to a study conducted among 559 care facility staff in January 2025, 35.1% of facilities reported such incidents in the past year, with 34.7% of cases taking over 10 minutes to discover the exit. Additionally, 47.1% of the staff expressed that identifying exits within five minutes is ideal, revealing a significant gap between the current capabilities and desired outcomes. Furthermore, 32.6% of participants recognized limitations within existing monitoring systems that rely solely on human vigilance or experience.

Prior to this extensive rollout, a pilot program was conducted in three facilities managed by Care Partner Inc., where the service's effectiveness was tested within the daily operations of caregiving. By focusing on practical aspects such as ease of operation, notification speed, reliability, and ease of ongoing use, the trial confirmed that the service meets operational standards. Instances of detecting potential exits before they occurred also demonstrated the preventative efficacy of the solution.

The staff participating in the pilot expressed profound relief regarding their psychological burden, emphasizing the practical advantages of the service. One participant noted, "Wearing the tag significantly changes the level of reassurance. I feel liberated from the anxiety of whether I can respond quickly, enabling a more calm approach to management." Another added, "The phone notification system is extremely practical. It ensures someone will always be on alert, and I would recommend it to other facilities."

1. Bluetooth Tag Detection: The service includes the use of Bluetooth smart tags that users carry, and receivers monitor the signals within the facility. If a user leaves a predefined area, the system immediately signals an exit.

2. Automated Voice Notification: When an exit is detected, an automated voice system instantaneously informs relevant parties via telephone, enabling substantial acceleration of the response process.

3. Simple Installation and Operation: The setup requires no specialized construction work, facilitating implementation by connecting the Bluetooth receiver to a power source. The design minimizes operational strains, particularly suited for the realities of care environments.

Following the official introduction in seven facilities in Tokyo, NEARIZE plans to accelerate deployment across various care facilities nationwide.
