Although Okinawa Prefecture, often associated with longevity, consistently ranked first for average lifespan in 1985, its standings have dropped significantly. As of 2020, Okinawa ranked 43rd for men and 16th for women nationwide. Alarmingly, mortality rates among the working-age population (35-64 years) are among the highest in the country, with the rate of individuals experiencing health check abnormalities exceeding 70%, marking Okinawa as the worst in Japan for 12 consecutive years.

One of the most pressing health concerns in Okinawa is the high death rate from brain and vascular diseases caused by high blood pressure. The prefectural government has identified combating high blood pressure as a top priority in initiatives such as Okinawa's "Health Okinawa 21" strategy, working toward the revival of Okinawa's health and longevity.
