A New Step Towards Collaboration in Community Healthcare and Welfare
On November 6, 2025, a significant event took place - the first meeting of the Setagaya Healthcare and Welfare Collaboration Council, organized by the
Hoyuukai Social Welfare Corporation, led by Director
Hiroshi Katori. This event is dedicated to enhancing cooperation between community healthcare and welfare services, aiming to build a better collaborative framework for the future.
Purpose of the Meeting - Creating Future Healthcare and Welfare Together
The primary goal of this council is to strengthen the ties between healthcare providers and the welfare system in Setagaya. By encouraging ongoing collaboration with local doctors who have consistently supported and guided the initiative, the council seeks to develop a sustainable model for community-based care. Feedback and suggestions from medical professionals will be taken seriously to further refine and grow the Integrated Community Care Model, ensuring that it meets the evolving needs of the local population.
Additionally, the meeting commemorates the 25th anniversary of the
Hoyuukai, where gratitude was expressed towards the physicians who have significantly contributed over the years. This acknowledgment works to foster even stronger relationships based on mutual trust and respect among all parties involved.
Future Outlook - The Path Ahead for Healthcare Collaboration in Setagaya
Looking forward,
Hoyuukai plans to intensify its efforts to improve the integration of healthcare and welfare services in Setagaya. This will begin with strengthening relations with local medical institutions and establishing a system for preventive support, using special nursing homes as foundational points. The organization is committed to advancing the Integrated Community Care Model through several upcoming projects, which include:
1.
Annual Local Training Project: This initiative aims to share the genuine voices of doctors with local residents, elderly care professionals, and various practitioners, thus establishing healthcare expertise as a vital community resource.
2.
Dementia Cafe Concept - 'Everyone's Health Cafe': By integrating a medical perspective into existing dementia cafes, this project aspires to elevate these spaces into community health hubs.
3.
Community Rehabilitation and Prevention Base at the Nursing Home Yamazaki: Aiming to open up the facility’s training room for community use, this initiative will work towards creating innovative rehabilitation and preventive care models utilizing the expertise of medical professionals.
New Facility - Kamiyouga no Ie (Tentative Name)
In June 2026, the opening of a new community-oriented special nursing home,
Kamiyouga no Ie (tentative name), is anticipated. This facility will uphold the quality of care and collaborative practices established over the years, evolving into a new hub that effectively integrates healthcare, welfare, and community support.
