New Blood Test for Tuberculosis: A Game Changer in Global Health



A recently unveiled study at the ADLM 2026 conference in Anaheim, California, reveals a promising development in the fight against tuberculosis (TB). This innovative blood test could accurately identify the presence of TB, which remains one of the world's deadliest infectious diseases. The research highlights the potential for this test to significantly impact the early diagnosis and treatment of tuberculosis, offering hope in efforts to reduce its transmission and ultimately save lives.

Dr. Sheng-Wei Pan, the lead author and physician in the department of chest medicine at Taipei Veterans General Hospital, emphasizes the urgency of prompt diagnosis in treating TB. He states, "Every week of delay in diagnosing TB matters—so early identification may allow treatment to begin sooner and potentially reduce transmission." According to the World Health Organization, approximately one-quarter of the global population is believed to be infected with the bacteria responsible for TB (Mycobacterium tuberculosis). Alarmingly, a percentage of these individuals will go on to develop the active form of the disease.

The traditional method of diagnosing TB often relies on sputum samples, which requires patients to cough up mucus from the lungs. However, not all patients are able to provide such samples, especially those in the early stages of infection or with low bacterial loads. This limitation underscores the urgent need for non-sputum-based diagnostic approaches, which is where the new blood test comes into play.

The researchers focused on a blood marker known as 6-kDa early secreted antigenic target (ESAT-6), produced by the TB bacteria. Despite previous research into this marker, many existing sensors lacked the sensitivity required to differentiate between active pulmonary TB and other lung conditions. Dr. Pan and his team employed a novel biosensor that overcomes these challenges, enabling a highly sensitive quantification of circulating ESAT-6 levels.

In their study, the researchers analyzed blood samples from 217 patients, some with active pulmonary TB infections while others did not have TB. The control group included various patients diagnosed with different respiratory diseases, as well as healthy individuals with known TB exposure. Results demonstrated that the ESAT-6 marker effectively distinguished between TB-affected and non-TB patients, revealing a progressive increase in blood concentrations of the marker corresponding to the severity of the TB infection.

Dr. Pan shared that this strong association remained robust even when adjusting for factors like age, sex, and diabetes presence, establishing ESAT-6 as a promising tool in TB diagnostics. Unlike markers that assess the immune response to infections, the ESAT-6 marker is derived directly from the TB bacteria itself, which explains its effectiveness in identifying active cases. The findings suggest that ESAT-6 performed better in distinguishing among different TB patient groups compared to two commonly used blood-based markers.

While the current study confirms the association of ESAT-6 with TB status, further research is required to validate its predictive capabilities in real-world scenarios. "We are currently planning a prospective study to further evaluate if this can be used in a real-world clinical setting," stated Dr. Pan.

Despite the ongoing necessity of sputum cultures for verifying TB diagnoses and assessing drug resistance, the introduction of the ESAT-6 blood test represents a significant advance. The test's potential to triage patients effectively, especially in situations where sputum-based diagnostics may be limited, could drastically improve outcomes in TB care.

The announcement was made during the scientific poster session at ADLM 2026, which runs from July 26 to 30 in Anaheim, California. This conference has brought together leading experts to share their latest research and technological advancements in laboratory medicine.
