On Lok PACE Achieves Special Recognition in Age-Friendly Health Systems Initiative



On March 10, 2025, On Lok PACE was officially recognized as an Age-Friendly Health System — Committed to Care Excellence. This accolade is a significant part of the Age-Friendly Health Systems movement spearheaded by the Institute for Healthcare Improvement (IHI). The organization aims to assure that health care for seniors aligns with their specific needs by assessing various elements of high-quality care.

Over the past few months, On Lok PACE has participated in a comprehensive evaluation process that focuses on a model referred to as the '4Ms': What Matters, Medication, Mentation, and Mobility. This framework is designed to enhance the quality of care that older adults receive, allowing for a more sensitive approach that prioritizes patient and family perspectives.

In total, since August 2023, more than 1,900 hospitals, clinics, and senior living facilities have been acknowledged for their proficient performance in senior health care standards under this initiative. On Lok PACE takes pride in being among the growing number of organizations implementing the 4Ms, contributing to a global outreach that has already touched the lives of more than 2.48 million seniors.

For over 50 years, On Lok has pioneered the PACE (Program of All-Inclusive Care for the Elderly) model, which is approved by Medicare and Medicaid. The initiative provides comprehensive support and services for eligible older adults, empowering them to age in place with the dignity and independence they deserve. On Lok embraces the 4Ms to ensure that all seniors have adequate access to care and resources that sustain their well-being and autonomy.

The commitment from IHI towards sharing valuable insights and best practices continues to strengthen as they aim to reliably implement age-friendly protocols across a range of healthcare settings, including emergency departments, ICU units, surgical wards, and primary care facilities.
