In an exciting development for community health services in Brooklyn, the Interborough Developmental & Consultation Center (IDCC) has been designated as a contracted partner in the Social Care Network (SCN) by Public Health Solutions' WholeYouNYC program. This strategic partnership will facilitate the connection of Brooklyn residents with essential health and social support services, targeting the social determinants that impact health outcomes.
What is WholeYouNYC?
WholeYouNYC serves as a comprehensive community resource network, designed to bridge the gap between healthcare and social services. The initiative focuses on improving health equity by placing individuals at the forefront of care—linking them with numerous resources catering to a variety of needs. These needs include food assistance, housing support, employment aid, legal assistance, and health services, all delivered in a culturally sensitive manner through navigators trained to handle diverse populations.
Incorporating over 300 community-based organizations, WholeYouNYC aids countless residents in navigating the complex landscape of available services across Brooklyn, Queens, and Manhattan.
Expansion of Community Outreach
The partnership comes at a critical time as IDCC launched its initiatives, such as the Winter Community Distribution events held in Brooklyn on December 16 and 17, 2025. During these events, IDCC staff engaged with local residents seeking winter supplies and facilitated direct connections with WholeYouNYC navigators. This hands-on approach not only provided immediate assistance but also paved the way for referrals through the Social Care Network, expanding access to staple support services like housing, food and nutrition assistance, and behavioral health care.

More than just a one-time interaction, the WholeYouNYC model emphasizes ongoing navigation and follow-through. Individuals registered during the community events will continue to receive care coordination, ensuring they have persistent access to the support they require.
