Habitat Health and Kaiser Permanente Partnership
Habitat Health has officially commenced its first Program of All-Inclusive Care for the Elderly (PACE) center through a collaboration with Kaiser Permanente in Sacramento, California. Located at 855 Howe Avenue, this center signifies a monumental step in providing comprehensive healthcare services tailored for older adults who are keen on maintaining their independence within their homes and communities.
Comprehensive Care Model
This innovative center presents a fully integrated healthcare model that not only covers primary and specialty care but also extends services such as prescriptions, social engagements, transportation, and home care, all consolidated under one facility. This framework allows older adults access to a diverse range of services in one place, enhancing their overall quality of life.
Brad Oglevee, the vice president and general manager of Habitat Health with Kaiser Permanente Sacramento, expressed enthusiasm regarding this initiative. He stated, "We are excited to offer a truly differentiated care experience to support the diverse needs of older adults who live here in Sacramento." His team believes that the convergence of Kaiser Permanente’s extensive expertise in complex care and Habitat Health’s committed interdisciplinary teams will not only provide peace of mind for older adults and their families but also foster a sense of community.
A Proven Approach
The PACE program, which has origins in California dating back to the 1970s, emphasizes community and home-based care. Research has consistently shown that such a model leads to reduced hospital admissions and lower rates of depression among older adults, while also improving their overall well-being. Furthermore, it aims to bridge health disparities, providing equitable healthcare access to all, irrespective of their background.
The Sacramento PACE center is the inaugural site of several PACE centers planned by Habitat Health and Kaiser Permanente across California, contingent on regulatory approvals. Dr. Jason Gritti, the medical director at Habitat Health with Kaiser Permanente Sacramento, highlighted the alignment of their work with the broader objective of enhancing the health outcomes of their members. He expressed, "The beauty of the PACE model of care is that it allows us to expand our scope of care to address older adults' medical, emotional, and social needs as a team."
