Rising Expectations for Healthcare CFOs



In an era of increasing pressure and complexity, healthcare chief financial officers (CFOs) are confronted by a significant readiness gap as they are expected to navigate their organizations through numerous enterprise decisions. A recent survey conducted by Deloitte highlights that while 73% of CFOs anticipate being deeply involved in vital decision-making areas, only 49% feel adequately prepared to step into those roles. This discrepancy underscores a concerning 24-point gap, indicating that many CFOs are struggling to align their expertise with the rising expectations placed upon them.

Key Findings from the Survey



The survey revealed that the most significant readiness gaps existed in areas critical to patient care and operational efficiency. For instance, decisions regarding patient experience, access, and affordability exhibited a staggering 33-point gap between expectations and preparedness. Despite 74% of CFOs asserting their involvement in such decisions, only 41% felt equipped to contribute effectively. In contrast, health systems showed a broader gap of 38 points versus health plans, which reported a 28-point gap. Furthermore, areas such as mergers and acquisitions (MA) and care model transformation presented gaps of 31 and 25 points, respectively.

Many CFOs remain optimistic about overall business performance, aiming for improvements in their organization’s margins. Nearly 60% of CFOs plan to enhance margins by at least 2 percentage points in the next two years; however, only 47% believe their organizations are prepared to manage the external pressures that could impact those margins. The study highlights critical factors such as access and affordability, which could pose challenges as CFOs attempt to maintain profitability while ensuring quality healthcare delivery.

The Promise of AI in Healthcare



Another focal point of the survey was the integration of artificial intelligence (AI) in healthcare finance. While the potential for AI to transform operations and provide financial insights continues to grow, the realization of these benefits remains a challenge. The survey reported that only 18% of CFOs from organizations deploying AI consistently measure its financial impact. Nevertheless, discussions surrounding AI have shifted slightly from a focus on anticipated results to a recognition of actual outcomes, with evidenced value narratives rising from 9% in 2023 to 19% in 2026. This indicates a slow transition from conception to practical application in the healthcare sector, but there remains significant work to fully realize AI's potential benefits.

Managing Drug Costs and Margin Pressures



The study also addressed the financial strains posed by escalating drug costs. A considerable 85% of finance leaders in health plans highlighted concerns over major impacts on their margins due to specialty drugs, but only 38% feel that their organizations are adequately prepared to manage these challenges. With these rising costs, CFOs must navigate complex financial landscapes while attempting to maintain service quality and access for patients.

Evolving Role of CFOs



The role of the healthcare CFO is poised to evolve from merely managing finances to orchestrating broader organizational value. As CFOs are increasingly tasked with integrating financial, operational, clinical, and strategic perspectives into their frameworks, their involvement in key decision-making processes must start much earlier. For organizations to thrive in this environment, they need to establish robust cross-functional planning and clear measures of value transformation.
