Koda Health and Medical Home Network Partner to Expand Advance Care Planning Access
In a significant move to enhance healthcare accessibility, Koda Health, an innovative platform utilizing AI for advance care planning (ACP), announced its new partnership with the Medical Home Network (MHN). This collaboration aims to broaden the accessibility and effectiveness of ACP services within selected Federally Qualified Health Centers (FQHCs) across the United States. With a focus on underserved populations, Koda Health's scalable, patient-centered solutions will be offered through MHN’s expansive network of healthcare providers.
MHN, recognized nationally as a public benefit corporation, is driven by a mission to transform care within the healthcare safety net and cultivate healthier communities. With partnerships existing over 80 FQHCs spanning eight states, MHN impacts the lives of more than 300,000 patients, including those on Medicaid, Medicare, and those eligible for dual coverage. This partnership will now further ensure that patients receive high-quality digital advance care planning services, crucial for effective decision-making in the face of serious illness.
Koda Health’s platform equips patients with the tools necessary to understand their healthcare options and facilitate informed choices that resonate with their personal values. This process allows patients to create legally compliant documents like advance directives or medical powers of attorney independently. Meanwhile, healthcare providers within MHN’s FQHC network will gain real-time access to their patients' care preferences, which is invaluable during times of critical decision-making. For the most vulnerable patients, Koda Health also provides ongoing expert support designed to guarantee that care aligns effectively with their preferences.
"This partnership brings advance care planning out of the abstract and into real, everyday care for patients who've historically been overlooked," stated Dr. Tatiana Fofanova, CEO and co-founder of Koda Health. She emphasizes the importance of making ACP a standard practice in healthcare settings to support those most in need.
Indeed, more than 30 million Americans rely on FQHCs for their primary care. Unfortunately, discussions regarding preferences in the context of serious illnesses or end-of-life situations are often avoided or postponed until it is too late. Alarmingly, studies indicate that nearly 25% of up to 30 million Medicare beneficiaries’ annual budgets are expended during their final year of life, often on treatments and interventions that patients may not have desired.
To combat these challenges, this partnership strives to revolutionize the way advance care planning is integrated into the healthcare framework at large. Koda Health has eliminated the traditional, labor-intensive methods typically associated with advance directive creation. By merging digital advancements with clinical expertise, these previously cumbersome processes are now streamlined and effective, resulting in patient-centric decision-making tools. Upon completion of these planning processes, Koda ensures that decision-makers and care teams gain immediate access to vital documents guiding treatment preferences.
This is especially pertinent for clinical settings where patients face chronic illnesses or acute healthcare emergencies, conditions commonly prevalent in the communities served by MHN and its FQHC partners. Dr. Desh Mohan, Chief Medical Officer and co-founder of Koda Health, remarked, "Advance care planning has traditionally been viewed as a privilege for the well-resourced; however, those in disadvantaged communities are often the ones who could benefit the most from it."
Motivated by extensive research demonstrating that high-quality ACP results in better alignment between patient goals and the care they receive, the collaboration is also poised to yield substantial cost savings. Koda Health has already observed positive trends through their data, which shows a 79% reduction in terminal hospitalizations, a 38% drop in ICU usage, and a 19% decrease in overall end-of-life care expenditures for patients availing themselves of their services.
Having dedicated over a decade to healthcare transformation, MHN upholds a commitment to value-based care, emphasizing holistic approaches that improve quality while reducing disparities in healthcare. Their partnership with Koda Health encapsulates the philosophy that healthcare decisions should reflect individual patient values and not merely clinical codes.
"At Medical Home Network, we believe high-quality care begins with understanding the needs of the populations we serve," emphasized Dr. Henish Bhansali, Chief Medical Officer at MHN. This partnership aims to ensure patients are empowered with tools and support for managing their care, successively paving the way towards dignity and equitable healthcare outcomes.
The partnership's implementation commenced in the second quarter of 2025, with plans to gradually extend to additional FQHC sites. Together, Koda Health and Medical Home Network aspire to set new standards for advance care planning within community health sectors, emphasizing empathy, equity, and significant impact across the healthcare landscape.
