SARCOA and Community Care Solutions Achieve National Recognition for Innovative Home Care Solutions
SARCOA and Community Care Solutions (CCS) recently achieved an impressive milestone by receiving dual accolades in the Federal Health at Home Challenge, a prestigious national competition organized by the Administration for Community Living under the U.S. Department of Health and Human Services. Both organizations were recognized not only as winners in their own right but also as integral members of an innovative national consortium designed to enhance in-home and community care services for older adults covered by Medicare and Medicaid.
Based in Dothan, Alabama, the SARCOA and CCS team, operating across 18 counties in southern Alabama and the Florida Panhandle, collaborated with partners like Statera Health and the Southeast Health Accountable Care Organization. This joint effort led to their selection as one of the ten Phase 1 winners across the country. Additionally, SARCOA and CCS participated in a broader winning coalition known as the National Consortium of Community Care Hubs for Health at Home, which spans eight states and is spearheaded by the Oregon Wellness Network in partnership with CCH Medical Group. SARCOA and CCS serve as the anchor for the consortium within their Alabama and Florida Panhandle markets.
The Federal Health at Home Challenge aims to assist older adults and individuals with disabilities to maintain their health and independence at home by reducing unnecessary hospital stays and managing overall care costs effectively. The judging criteria focused on the ability of the participating teams to deliver vital health services, including navigation assistance, care coordination, smooth transitions of care, evidence-based health promotion initiatives focused on preventive measures like fall prevention, and nutritional support.
Impacts on the Wiregrass and the Panhandle
The awards garnered by SARCOA and CCS pave the way for an expanded and improved model of care that both organizations have been diligently constructing. This model integrates a detailed network of community-based organizations directly linked to healthcare facilities, physicians, and insurance programs. Existing partnerships, particularly with the Southeast Health Accountable Care Organization and Blue Cross and Blue Shield of Alabama, will be instrumental in reaching more seniors across the region. The outcome is expected to include improved post-hospitalization support for older adults, tailored care plans facilitated by nursing and community health professionals, community-focused fall prevention programs, and access to home-delivered nutritional aid.
With the Health at Home Challenge recognition in hand, SARCOA and CCS are set to extend their reach into 16 additional counties in Alabama, further solidifying their commitment to comprehensive community care.
Collaboration is a vital aspect of this initiative, as SARCOA and CCS plan to work closely with local healthcare systems, accountable care organizations, and managed care plans in the region. Their overarching goal is to enhance health results and streamline costs. Anticipated outcomes include increased healthy living days for residents and improved performance based on HEDIS metrics, which health entities utilize to evaluate the quality of care provided.
Residents will access services through referrals initiated by local hospitals and primary care practices, creating a seamless connection between various healthcare pathways. These will encompass hospital-to-home transitions, primary care and ACO referrals, nursing home-to-community transitions, fall prevention programs, and support for managing social isolation and depression—all delivered in a collaborative approach.
Innovative Technologies for Enhanced Care
Within the framework of this national consortium, all member care hubs will make use of a unified electronic medical record system, the AthenaOne, integrating advanced artificial intelligence for thorough population health analysis. Additionally, Community Care Solutions will leverage the capabilities of Cumulus Care, another AI-backed tool designed to enhance resource accessibility and chronic health management.
These innovative platforms will empower care teams to detect at-risk individuals earlier, guide community health workers effectively, and quantify important metrics such as days spent at home versus care costs.

Both winning teams are set to advance to Phase 2 of the Challenge, running from August 2026 to July 2027, where they will implement their innovative strategies at both local and national levels. Up to five teams will be selected in summer 2027 for further scaling up their initiatives, with an aim to transform home care across the country.
