Eloxx Pharmaceuticals Commences Phase 2 Study for ADPKD Disease Treatment with Clinical Leadership Announcement
Eloxx Pharmaceuticals, Inc. (Nasdaq: ELOX), a company dedicated to advancing innovative treatments for rare kidney diseases, has made significant strides recently by finalizing the design of its 52-week Phase 2 EXPRESS study intended for individuals suffering from nonsense-mutation autosomal dominant polycystic kidney disease (ADPKD). This study aims to assess the efficacy of exaluren, a potential therapeutic agent designed to address the genetic mutations leading to kidney dysfunction in afflicted patients.
The EXPRESS trial is set to follow a rigorous design that includes the participation of approximately 60 adults, all of whom possess genetically confirmed mutations in either the PKD1 or PKD2 genes. Participants will be randomized in a 1:1 ratio to either receive exaluren or a placebo over a duration of 52 weeks. The primary goal of the trial is to measure annualized changes in height-adjusted total kidney volume (htTKV), as determined through MRI scans conducted at baseline, as well as at Weeks 28, 36, and 52. The results are expected to offer valuable insights into the long-term effects of the treatment and contribute to the understanding of its safety profile.
Eloxx's determination to establish a robust clinical study comes after extensive discussions with regulatory authorities and the advice of key opinion leaders in the field. Positive feedback from the FDA indicated that a notable and lasting impact on htTKV, combined with supporting evidence on kidney function, could justify the potential for Breakthrough Therapy Designation. The FDA has identified htTKV changes as a potential surrogate endpoint that could accelerate the approval process for treatments targeting ADPKD, which means that success in the EXPRESS trial could pave the way for faster access to this important treatment for patients. First patient dosing is anticipated in the third quarter of 2027, with topline results expected in the first half of 2029.
In conjunction with the study announcement, Eloxx Pharmaceuticals has also appointed Dr. T.R. Srinivas, M.D., as Senior Vice President and Chief Medical Officer. Dr. Srinivas brings an extensive background in nephrology and clinical research, having previously served as a leader within Novartis's U.S. Medical Affairs team for ADPKD product development. His appointment underscores Eloxx's commitment to enhancing its clinical leadership and expertise in driving the company’s efforts in tackling rare kidney diseases.
Dr. Srinivas expressed the pressing need for effective therapies targeted at patients experiencing early kidney failure due to nonsense mutations, stating that existing treatments do not address the fundamental genetic drivers of the disease. Exaluren represents a unique approach aimed at restoring the critical proteins that these mutations prevent from being produced.
As part of its progressive initiatives, Eloxx has also secured orphan drug designations for exaluren in ADPKD within the U.S. and across Europe. The designation further highlights the potential impact of exaluren in addressing unmet medical needs and providing new therapeutic options to patients affected by this debilitating condition.
Nonsense-mutation ADPKD is marked by the presence of mutations in the PKD1 or PKD2 genes, leading to the formation and enlargement of cysts in the kidneys. This condition affects an estimated 160,000 to 200,000 individuals in the U.S., resulting in progressive decline of kidney function and increased risk of kidney failure. With about 26% of the mutations classified as nonsense mutations, the need for effective treatments has never been more critical.
Exaluren seeks to facilitate the readthrough of these premature stop codons, ultimately promoting the synthesis of full-length, functional proteins, thus providing hope for a new pathway in treating kidney diseases linked with nonsense mutations. Eloxx Pharmaceuticals, through its dedication and innovative approach, aims to redefine the landscape of treatments for rare kidney conditions, ensuring that patients have access to the therapies they need.
