The National Consortium of Community Care Hubs for Health at Home includes eight community care hubs from various states, all designed to promote in-home and community care for older adults. Among these hubs, TLC-MD occupies a vital role, anchoring the Maryland and Washington, D.C. markets alongside Freedmen's Medicine, a dedicated primary care practice specializing in home-based care for adults facing chronic conditions.
This national competition recognized teams for their capabilities in delivering navigation, care coordination, and evidence-based health promotion programs aimed at reducing hospitalization rates and overall care costs. TLC-MD and its partners were among the ten winning teams selected for their vision and potential to make a sincere impact.
The Challenge's Positive Impact on Maryland and D.C.
