BigHeart Health Launches Innovative Program for Illinois Medicaid Members to Access Comprehensive Care

BigHeart Health Unveils 'My Health. My Power.' Program

In a bold move to enhance healthcare access for underserved populations, BigHeart Health, a Chicago-based virtual care provider, has officially launched its pioneering initiative, 'My Health. My Power.' This program is designed specifically for Illinois Medicaid members, offering them vital resources and support to help maintain their health before issues escalate into serious conditions.

Set against the backdrop of increasing healthcare challenges in the wake of Medicaid rule changes, this program aims to fill gaps in preventive care and promote healthier living. On July 10 and 11, BigHeart Health hosted enrollment fairs at a local Walmart in Chicago, inviting eligible members to join the program and access a wealth of resources including healthy recipes and wellness education.

A Comprehensive Approach to Healthcare

'My Health. My Power.' is more than just a healthcare program; it represents a modern, community-oriented approach to patient support. The initiative will actively engage over 400,000 Medicaid members in Illinois, addressing stark healthcare disparities and improving health outcomes in collaboration with Managed Care Organizations (MCOs).

Participants in the program will receive custom-tailored support through a Personal Healthcare Consultant. This consultant will assist in linking members to necessary healthcare services such as primary care providers, specialists, and mental health resources while also facilitating access to preventive screenings and chronic disease management programs.

Key Benefits for Members

Eligible Medicaid members will enjoy numerous benefits at no cost, including:
  • - Personalized Guidance: Dedicated healthcare teams help members navigate their health journey effectively.
  • - Access to Specialists: Support in locating doctors, mental health providers, and preventive services.
  • - Chronic Disease Management: Coaching on managing conditions like diabetes, obesity, hypertension, and kidney disease.
  • - Digital Resources: Comprehensive wellness materials focused on nutrition, emotional health, and stress management.
  • - Community Connections: Access to essential services covering food, housing, utilities, and employment.

This program prioritizes outreach to the communities most in need, including those facing chronic health issues, young families, and LGBTQ+ members. BigHeart Health aims to connect with these individuals through collaborations with local nonprofits, community organizations, and churches, ensuring that the services reach those who need them most.

A Call to Action

Cheryle Jackson, CEO of BigHeart Health, emphasized the urgency of the initiative: “In the midst of Medicaid cuts and changes, preventive care has never been more important. Our goal is to empower individuals with the knowledge and resources they need to take control of their health.”

Enrollment in the program is straightforward and can be done online at myhealthmypower.com or by calling 312-210-7820. By offering a blend of technological support and community engagement, BigHeart Health is committed to building healthier futures for Illinois Medicaid members.
