The CVS Health Foundation recently made a significant announcement, revealing a funding initiative totaling $2 million directed towards the Hispanic Health Council's Family Wellness Center and its partner organizations in Hartford, Connecticut. This funding is aimed at tackling the critical barriers that hinder access to health care for individuals suffering from chronic health conditions, thereby improving overall health outcomes in the community.

In light of this reality, the CVS Health Foundation tailored its Health Zones initiative to bolster the Family Wellness Center's outreach and services. Partner organizations involved in this initiative include Mental Health Connecticut and Villages for Families and Children, offering a comprehensive suite of services that encompasses preventative care, nutritional counseling, mental health support, and community health education.

As part of this initiative, the funding will also allow the Hispanic Health Council to employ three new patient navigators. These professionals will assist individuals in navigating the complexities of the health care system, ensuring they receive the appropriate care and resources they need. In conjunction with the funding announcement, CVS Health organized a free health screening event at the Family Wellness Center, part of its ongoing Project Health initiative. This event offered attendees on-the-spot consultations with healthcare providers, alongside a directory of local health resources. Last year, CVS Health had a significant impact in Hartford, conducting 23 Project Health events that screened over 450 participants and facilitated more than 1,500 health screenings throughout the year.
