Exploring the Role of Serial LDH Kinetics in Glioblastoma Assessment
In an exciting development from Alpheus Medical, recent data unveiled at the ESMO Congress 2026 highlights the potential of serial lactate dehydrogenase (LDH) kinetics as a novel, minimally invasive biomarker for monitoring disease activity in patients with glioblastoma (GBM). Conducted in Madrid, Spain, during the prominent oncology congress, the study draws attention to an innovative approach that may elevate traditional imaging methods in assessing disease status.
Alpheus Medical is at the forefront of research, aiming to revolutionize glioblastoma treatment through its Porphyrin Metabolite Activation (PoMA™) strategy. This recent investigation, which represents the first human clinical study involving PoMA™, was encapsulated in the poster titled "LDH Kinetics as a Biomarker of Disease Activity in Glioblastoma Treated with Porphyrin Metabolite Activation (PoMA)." The study proposes an alternative perspective by focusing not exclusively on static LDH values, but rather on how these values fluctuate over time and correlate with both clinical and radiographic disease status.
The exploratory analysis involved 15 patients diagnosed with MGMT-unmethylated recurrent glioblastoma and receiving treatment with PoMA™. A critical finding is that stable or declining LDH levels were observed in patients who displayed no clinical signs of disease progression. Conversely, those patients who experienced clinical deterioration uniformly exhibited increasing LDH levels throughout their treatment course. Notably, some patients suspected of having disease progression based on imaging studies showed stable LDH levels, indicating that serial LDH changes might yield complementary insights when imaging results can be ambiguous.
Dr. Vijay Agarwal, CEO of Alpheus Medical and a neurosurgeon, remarked, "Cancer biomarkers are often interpreted as snapshots in time, and imaging of glioblastoma can pose challenges due to treatment-related changes mimicking disease progression. Our goal was to understand whether monitoring the trajectory of LDH could provide additional clarity. The findings suggest that consistent increases in LDH are linked to clinical deterioration, while patients without progression typically showed stable or declining LDH levels."
Further expanding on the analysis, it was noted that consecutive LDH increases or significant initial spikes in LDH appeared to be more indicative of disease status when compared to smaller fluctuations. This could potentially reshape how clinicians interpret biomarker data in the context of glioblastoma treatment and monitoring, helping bridge gaps where traditional imaging may fail to provide clear answers.
As Alpheus Medical pivots towards prospective evaluations within a larger patient population, the aim is to deepen the understanding of how these serial LDH values are intertwined with clinical conditions and imaging results. The promise of harnessing LDH kinetics as a biomarker could revolutionize patient monitoring protocols, offering oncologists a more precise tool for gauging treatment efficacy and disease progression.
The Mechanism Behind PoMA™
The therapeutic approach known as Porphyrin Metabolite Activation is designed to leverage the unique properties of protoporphyrin IX (PpIX), which preferentially accumulates in tumor cells after the administration of 5-aminolevulinic acid (5-ALA). This drug-led method employs low-intensity defocused ultrasound to activate PpIX, potentially enabling effective treatment across the cerebral hemisphere. Currently, PoMA™ is undergoing rigorous clinical investigations focused on both newly diagnosed glioblastoma cases and rare butterfly glioblastoma variants.
