Leading MSSP ACO for Long-Term Care Reveals Name Change to Align with Expanded Services
Significant Rebranding for Enhanced Care
In a strategic move to better reflect its evolving scope of services, Sound Long-Term Care Management (SLTCM) has announced its rebranding to Sound Accountable Care. This new identity marks a pivotal moment for the organization, illustrating its commitment to providing comprehensive, patient-centered care. The organization has significantly transformed the dynamics of long-term care since its inception. In merely three years of operations, Sound Accountable Care has excelled in setting benchmarks within the long-term care industry, specifically for the Medicare Shared Savings Program (MSSP) for facility-based beneficiaries. Demonstrating its capacity to deliver impactful results, in just its second performance year, the organization aided Medicare in realizing nearly $114 million in shared savings.
